At the time of the request, please provide: the name of the product, where it was purchased, the Lot Code # on the package if available, and the price at time of purchase. You will also need to provide your full name and shipping address. Your personal information will not be used for any other purpose other than in completing your refund or exchange. You can request either a replacement product at no cost, or a full refund. For a refund, a check will be mailed to you. For an exchange, the replacement product will be shipped directly to you at no cost.
